evaluating the association of a vegetarian diet with measures of nutritional status in healthy children. Consistent with previously published literature 2 , there does not appear to be any meaningful adverse health outcomes associated with a vegetarian diet in children. The investigators' decision to place emphasis that a vegetarian diet is associated with an increased risk of being underweight is misleading, and potentially dangerous.

The authors failed to highlight that while being vegetarian was associated with an increased relative risk of being underweight, the absolute difference was small a 3 percent absolute difference , and the clinical relevance of this small absolute difference is unclear when the number of patients in each group are not well balanced in number or ethnicity.

Furthermore, the authors failed to acknowledge in their discussion that larger prospective cohort studies conducted in adults found a vegetarian diet is associated with a reduction in all-cause mortality 4. Another interpretation of this study could be that since a vegetarian diet is not associated with deleterious outcomes in children, its implementation can be safely encouraged due to its potential long-term benefits.

The population health benefits from the promotion of these habits in childhood are likely to supersede any adverse outcomes associated with striving to attain an arbitrary standard that is color-blind to inherent ethnic differences seen in growth and development.

I would further suggest that even if there are no health benefits associated with a vegetarian diet, as a community dedicated to the promotion of health, we should not be blind to the benefits of a plant-based diet on planetary health including: a reduction in green-house emissions, its potential to contribute to the security of the global food supply 5 , and a reduction in suffering of non-human animals.

A more appropriate clinical approach would simply be to advocate and discuss appropriate nutrition for all children based on their specific needs accounting for cultural values and family preferences without the need the characterize a vegetarian diet as specifically being problematic.
